Description:

As the Registered Dietitian you will provide initial nutritional assessment, treatment plans, and on-going nutritional care for HIV+ clients in an outpatient setting. You will provide additional support services for other programs such as the Food Voucher Program, the Lunch and Learn Programs, Diabetes Education Programs, and the Employee Wellness Program.


Daily, you will contribute to the Prisma Community Care mission of providing affirming and inclusive services to promote well-being and advance health equity for diverse communities and all those seeking compassionate care - especially people of color, 2SLGBTQIA+ and Queer individuals, and those affected by HIV.


What your day will look like at Prisma Community Care:

  • You will conduct initial, annual, and follow-up assessments for all eligible Ryan White HIV+ clients including:

Review client's current and past statistical information

Review physical symptoms contributing to weight gain or loss

Review laboratory values if they are available from client or their primary care provider

Review general health and lifestyle details, diet history/nutrition food frequency

Review any dietary supplements and provide recommendations

Provide assessments of body composition- conduct BIA analysis, when able

Provide referrals to food pantry and/or subsidized food programs

Create and communicate nutritional individual treatment plans that achieve measurable and attainable goals

  • You will collaborate with primary care providers, case managers, and other service professionals in the care and treatment of clients.
  • You will methodically and meticulously chart medical data discovered in the consultative process.
  • You will conduct services in accordance with Maricopa County department of health care mandates, Ryan White Part A funding source contract provisions, Prisma Community Care policies and procedures, and Academy of Nutrition and Dietetics.
  • You will provide education and training for clients and other professionals, review literature journals and professional literature to keep abreast of new developments.
  • You will assist with food voucher clients as needed.
  • You will assist in the vitamin shop to directly serve clients and eliminate wait time, when applicable.
  • You will assist in Men's and Women's Day Lunch and Learn, including catering, survey results etc.
  • You will assist in Diabetes Education Programs, when applicable.
  • You will be the lead person for Employee Wellness Program, when applicable.

Requirements:

Who we're looking for:

  • You've graduated from High School or completed your GED.
  • You currently possess a Registered Dietitian credential.
  • You are familiar with HIV or other complex disease states and nutritional complications of comorbid conditions.
  • You are able to understand and communicate with people of varying education levels and cultural diversity.
  • You demonstrate a positive attitude and compassion while working in a team-oriented setting and independently and team- oriented can work independently.
  • You are able to manage time effectively.
  • You demonstrate the ability to collect, compile and analyze medical information.
  • You're proficient in Microsoft Office suite and competent in electronic medical record software programs.
  • You have demonstrated cultural competency to professionally and respectfully serve the 2SLGBTQIA+ communities.

  • You have at least one year of experience in an outpatient or clinical inpatient/hospital setting.
  • You are bilingual in English and Spanish.

Environmental Factors, Physical Requirements, and Compliance

  • Work primarily in a climate-controlled environment with minimal safety/health hazard potential.
  • Office environment, exposure to computer screens for lengthy periods of time.
  • The position requires extensive use of computers using a standard computer keyboard.
  • This position may require evening and occasional weekend work.
  • While performing the duties of this job, the employee is regularly required to walk, sit, stand; use hands, talk, and hear. The employee is occasionally required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, and ability to adjust focus.
  • This position may require the employee to occasionally climb, balance, stoop, kneel, crouch, bend, or crawl.
  • Currently have, or be able to obtain within 90 days of employment, a valid Fingerprint Clearance Card
  • Currently have, or are able to obtain within 7 days of employment, a clear TB test
  • Currently have, or are able to obtain within 7 days of employment, a current flu vaccination
  • Currently have, or are able to initiate within 7 days of employment, a Hepatitis-B primary vaccination series
  • Currently have, or are able to obtain within 14 days of employment, a CPR certification
