Location: Arun Villivalam, MD

Position: Physician Assistant

Contracted hourly rate with conversion to full time @ 32 hours as noted below

start at 8 hours (split into 4 hour blocks) / week

target 24 hours / week in 3-6 months

32 hours in 6-12 months

Responsibilities would be patient care at the provider level including but limited to:

patient visits - up to average 20 / day, initially start at 1 patient every 30 minutes

reviewing labs / studies

coordinating care

completing all notes same day so they can be sent for billing

Benefits @ < 32 hours / week:

hourly rate of $85 / hour

Malpractice included (tail cost would be split 50/50)

Benefits @ 32 hours per week 32 + hours / week is full time

hourly rate of $85 / hour

Dictation software provided

CME $2000 / year

15 days sick time / PTO

No call

malpractice tail covered after 1 year of employment full time

profit sharing possible

may contribute to 401k

Coverage of license fees for DEA / NP licensure / RN licensure / NP Furnishing when @ 32 hours / week
