Full Job Description
Responsible for assisting the Integrated Health Services team by identifying, scheduling and coordinating patient care opportunities. The Integrated Health Services Assistant assists with bringing patients into exam rooms and preparing them for the Behavioral Health and/or Registered Dietician provider(s), under the direction of the provider, by assisting the patient to complete pre-visit screeners and providing language interpretation services as needed.
Essential Functions
- Schedule and coordinate care for patients to consult with a registered dietitian and/or behavioral health consultant. Patients may be scheduled for future appointments or be referred internally for same day care by a medical provider during scheduled office visits. Assist with rescheduling patients as needed.
- Identify patients who may benefit from integrated health services in a systematic way prior to medical provider visits.
- Provide interpretation services (English and Spanish) for limited English proficient patients to ensure accurate and effective communication between health/integrated care team and patient.
- Coordinate appointments between integrated health services team and medical team of patients identified by arriving patients on schedule and informing integrated providers of location of patients.
- Prepare patients for visits with the integrated health services team by bringing patients to room and providing pre-visit screeners.
- Knowledgeable in basic level Microsoft programs.
- Assist with integrated health service special projects by presenting/attending to staff meetings and huddles and managing tasks within the electronic health record.
- Complies with all applicable infection control and safety procedures.
- Takes and answers all voicemail and written messages in a timely manner.
- Interacts politely and as a team player with all MPHC employees and public.
- Must be able to embrace differences among people and able to interact with internal and external contacts in a culturally competent and respectful manner.
- Performs other duties as assigned.
Position Qualifications
Minimum Qualifications:
- High School Diploma or GED
- Must be bilingual and able to interpret for Spanish-speaking patients.
Preferred Qualifications:
- Completion of Medical Assisting Certificate program, OR
- Associate Degree in Psychology, Sociology, Healthcare or related field
- Experience working in a healthcare setting
- Certified Interpreter
